Facilities Request Form

In order to accommodate the many facilities requests we receive, and enter them on our Main Calendar we need each request to be submitted in writing. Please complete the attached form for each events. Include

  • Time and date
  • Specific room requests
  • Resource requests such as nursery and/or security
  • Staff support - advertising, room set-up, meals or refreshments
  • Contact person's name, email and phone
  • Recurring events date range

It is also important that offsite events have a form so that we can avoid scheduling conflicts between events. 

Attached is a PDF if you would like to print your own forms, or you may pick one up at the Front Office downstairs. 


Please put your completed form in Joy’s mail tray in the drawer in the Front Office, drop it by Peggy’s office, or email Joy or Peggy.  


Thank you for working with us to ensure that all events are scheduled correctly and details noted for our Main Calendar. Any changes to your event must be submitted in writing.
